Patient Portal

If you are a Community Medical Center, Community Children's, Community Physican Group or Community FirstCare patient, you can access your medical records and communicate with your providers online. Please note for patients ages 12-17, the portal will only display limited information in order to protect patient privacy. Patients and their proxy users (parents) will have access to see immunizations, the ability to message providers and request appointments.

New to online patient services?

Sign-up Online
You will need your medical record number (MRN#) to sign-up online. This can be found on a billing statement listed as Account #, on a recent visit summary, contact your medical provider's office or call the number below.

Third-Party Applications 

It is now possible to access your health information through approved third-party applications, such as those used on a smartphone. Depending on the chosen application, you may be able to access information such as your medications, allergies or lab results all in one app location. Have an application you would like to use?
