All patients can login to our patient portal, MyCommunity Health, to review your medical records.
MyCommunity Health is a patient portal that connects you to a portion of your personalized health information from the privacy of your home at any time, day or night using a secure, online patient portal.
- Electronic communication with care team
- Request appointments
- Request medication refills
- View immunization records and medication list
- Obtain lab and radiology results
Please note for patients ages 12-17, the portal will only display limited information in order to protect patient privacy. Patients and their proxy users (parents) will have access to see immunizations, the ability to message providers and request appointments.

Locate medical record number (MRN#) on a past billing statement (Account #) or recent visit summary.
Ask us at your next visit or stop by Guest Services at the main hospital entrance.

Apple Health App
The Health app on iPhone & iPod touch acts as one centralized hub for all your health data from various third-party apps and devices such as your Apple watch, run or step trackers, sleep trackers, food diaries, etc. Community Medical Center supports health records on the Health app for Apple iPhones & iPod touch devices. This will enable patients to view important data such as immunizations, lab results, medications and vitals directly in the Health app.
To connect your medical records to your Health app. locate the app on your device > select your profile > select Health Records > search for "Community Medical Center" > login with your patient portal email and password.
